Scheduling an Appointment

An appointment can be obtained by calling the Counseling Center at (949) 824-6457. In the "intake appointment," a client will meet with a counselor to discuss his or her concerns and identify Counseling Center services that might be appropriate.

If your situation is an emergency you may request to be seen immediately. Just call or drop-in between 8:00 a.m. and 5:00 p.m.


New Client Information

Prior to your first appointment you will be asked to complete the following intake forms. For your convenience, you may print out each form or come by the Counseling Center and pick them up prior to your intake appointment. Most people take 30 minutes to complete these forms. Please arrive 15 minutes early to your intake appointment.

This fact sheet contains information about the Counseling Center, its services, and what you should expect.

Student Information (pdf)
Gives us an opportunity for you to tell us about you. If you are uncomfortable with any question, please feel free to skip it. This information becomes a part of your Counseling Center file. It is subject to the same principles of confidentiality outlined in the Informed Consent for Counseling Services.

Group Programs Brochure (pdf)
Contains detailed description of groups offered at the Counseling Center as well as information on how to join groups.

Group Information List (pdf)
Provides an opportunity to state groups you are interested in joining.  Please complete and bring this form to your first appointment.

Notification of Privacy Practices (pdf)
Please read and complete this form.

Informed Consent (pdf)
This must be read completely and signed in order for the counselor to see you.

Parental Consent for Treatment of a Minor (pdf)
If you are under age 18 years please have your parents sign the above form..


After Your First Appointment

After your intake appointment, you and your counselor will decide what services will be most helpful to you. This may include:

Counseling sessions at the Counseling Center

OR

Referral to Student Health Center

OR

Referral to other services as deemed appropriate.

In the case of being referred to an outside service you may need the following insurance information
